Job Description
Happier At Home Care is a non-medical home care agency located in Bucks County. We are seeking Full and part-time LPN's or MA's with experience in the home care field. Our clients are located throughout Bucks and Montgomery County.
Responsibilities
- Assessments of new clients
- Facilitate client-caregiver meet and greets
- Document visits and changes in WellSky
- Address any health issues or concerns
- Manage care plans and medication lists
- Avalible to cover call out and no show shifts
- Train caregivers on tasks pertaining to each individual client
- Client, caregiver and family reconciliation and deescalation
- Meeting with clients to go over care plans and any changes in care
- Random and announced visits, minimum once a week
- Hospitalization visits and post-hospitalization visits
- Communication with the family
- Document visits in client electronic chart for Happier At Home Care
Qualifications
- Pass background check
- Excel in a high-speed work environment and multitask
- Able to work in a team
- Adequate technological skills
- Driver’s license
- Knowledge of medical terminology
- Strong interpersonal skills with the ability to empathize with patients from diverse backgrounds
- Excellent organizational skills and attention to detail to manage multiple cases effectively

Pay
$22.00 - $25.00 per hour
