About the role

Skilled Home Health is seeking a Licensed Vocational Nurse (LVN) to join their team. The position involves planning and delivering nursing care in compliance with company, federal, state, and local standards, guidelines, and regulations. The ideal candidate must have a nursing degree, an unencumbered license to practice as an LVN, proficiency with electronic medical equipment, knowledge of nursing/medical practices and procedures, and experience in home health settings.

Responsibilities

• Aiding and understanding to the family in a hospice care setting
• Driving to patient’s homes in the Woodland Hills SF Valley area
• Planning and delivering nursing care in accordance with current company, federal, state, and local standards, guidelines, and regulations
• Ensuring the highest degree of quality care and dignity is maintained at all times
• Providing direct care to multiple patients
• Experiencing in hospice environment

Requirements

• Nursing degree from an accredited college or graduation from an approved LVN program
• Unencumbered license to practice as an LVN
• Proficiency with electronic medical equipment
• Knowledgeable of nursing/medical practices and procedures, and/or terminology, laws, regulations, and the guidelines that pertain to home health
• Experience in home health setting
• Willing and able to use your own vehicle to drive to patient’s homes
• Current driver's license and proof of insurance required
• On call availability
