Responsibilities

• Conduct comprehensive assessments of patients.
• Develop and implement personalized care plans.
• Administer medications and treatments.
• Educate patients and families on care and health management.
• Work with an interdisciplinary team to coordinate care.
• Document all care in the electronic medical record (EMR).
• Support community outreach initiatives.

Qualifications & Skills

• Compassionate and independent with strong clinical assessment skills.
• Proficient in medication administration and patient education.
