Field Registered Nurse - ACT Program

About the role

The RN serves as a member of the ACT team, providing comprehensive care to clients with Substance Use Disorder (SUD), Substance Use Disorder (SPMI), and those requiring rehabilitation and support services. Services are provided across the city, primarily in the community.

Responsibilities

• Carry a caseload and conduct initial and ongoing nursing assessments.
• Conduct weekly visits to clients in homeless shelters and their homes or other accessible locations.
• Develop and implement a treatment plan based on clients' needs.
• Avoid and educate clients about health conditions and symptoms.
• Complete nursing assessments and health monitoring assessments within specified timeframes.
• Follow and implement medical treatments and medication orders from psychiatrists or PNP.
• Administer prescribed and routine medical treatments, assist with self-administration of medication, and evaluate for side effects or adverse reactions.
• Promote healthy lifestyle changes through a harm reduction framework, with shared decision-making.
• Coordinate and deliver medications to clients, manage medication counts, and follow up with pharmacies.
• Make referrals, schedule appointments, and accompany clients when necessary.
• Visit hospitalized clients to coordinate care and provide advocacy.
• Assist with follow-up care to ensure complete recovery.
• Act as a liaison between external medical health care providers, shelters, and community organizations.
• Provide health education and communicate good health practices to clients.
• Train staff in these practices to provide continuous health support to clients.
• Facilitate groups and respond to medical emergencies as needed.
• Perform necessary charting and documentation as required by internal practice and external regulatory bodies.
• Monitor and communicate lab results and medication changes to relevant healthcare professionals.
• Participate in interdisciplinary mental health team meetings.

Requirements

• RN or BSN in nursing and New York State registration.
• Experience in a residential setting or in mental/behavioral health field preferred.
• Comfortable using electronic health records, Microsoft Word, Excel, and Outlook.
• Manual dexterity necessary for handling medical equipment and performing clinical procedures.

• Ability to navigate various environments, including tight spaces, while maintaining focus on patient care and safety.

Skills

• Strong communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Knowledge of harm reduction principles and shared decision-making practices.
