Field Physical Therapist - Home Health

Responsibilities

• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of physical therapy.
• Provides the appropriate level of clinical direction for Physical Therapy Assistants.
• Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patients' needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
• Manages the activities of the multi-disciplinary team providing care to the patient.
• Maintains open communication with all disciplines regarding the patient's care.
• Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
• Interprets agency policy to patients/families.
• Reviews patients' visits weekly and provides a plan to the Home Care Supervisor and scheduler, including anticipated weekend visits.
• Informs the scheduler of any schedule changes and communicates caseload and patient care issues to the Home Care Supervisor.

Requirements

• Bachelor's Degree in Physical Therapy.
• Physical Therapist State of Florida License.
• One year Physical Therapist experience.
• BLS (Basic Life Support).
• Driver's License (Florida driver's license is required within 30 days of hire).

Qualifications

• Preferred Master's Degree in Physical Therapy.

Schedule

Full-Time, Exempt. Shift Hours: 8:00 a.m. - 5:00 p.m., Monday-Friday. Weekend Work: Every 3rd On Call: No Home visits in assigned service area: Largo, St. Petersburg, Pinellas Park - surrounding areas assigned by zip code.
