Responsibilities
- Perform comprehensive nursing assessments (admissions, recerts, condition changes)
- Develop and manage individualized care plans aligned with physician orders
- Monitor patient progress and proactively address changes in condition
- Ensure all care meets clinical, regulatory, and company standards
- Maintain accurate, timely documentation in AlayaCare (or similar EMR)
- Provide hands-on leadership to LPNs and RNs in the field
- Conduct ride-alongs, chart audits, and competency validations
- Deliver real-time coaching and professional development
- Serve as the go-to clinical expert for complex patient situations
- Complete RN admissions and communicate care needs across teams
- Collaborate with scheduling, billing, and operations for smooth onboarding
- Build relationships with hospitals, physicians, and referral partners
- Maintain adherence with DC, MD, and VA home care regulations
- Support audits, accreditation readiness, and quality initiatives
- Uphold strict HIPAA and documentation standards
- Maintain multi-state RN licensure
Requirements
- Active RN license (VA and/or MD required; DC within 90 days)
- 2+ years of clinical nursing experience
- Prior experience supervising or mentoring clinical staff
- Valid driver's license + reliable transportation (field role)
- Strong communication and organizational skills
- Comfort working independently across a multi-site territory
Preferred Qualifications
- Experience in home health or high-acuity/skilled care
- Associate's degree in Nursing (or higher)
- Experience with EMR platforms like AlayaCare
Work Environment
- Primarily field-based—serving patients in their homes
- Regular travel throughout DC, Maryland, and Northern Virginia
- Flexible scheduling, including occasional evenings
- Physical activity consistent with home health (standing, bending, lifting up to 20 lbs.)
