About the Role
The Field Nurse is an integral team member of the FreedomCare Licensed Home Care team. This position requires 5 days per week of fieldwork throughout the greater Albany, NY area. You will assess patients' current service needs and reassess needs upon significant changes in health status.
Responsibilities
- Conduct initial assessments of clients to determine care needs and develop individualized care plans.
- Train and supervise non-skilled caregivers in providing personal care, companionship, and assistance with daily living activities.
- Monitor clients’ overall well-being and report any changes to the appropriate healthcare professionals.
- Educate clients and their families on effective care strategies, safety measures, and daily living skills.
- Ensure compliance with agency policies, state regulations, and best practices in non-skilled care.
- Maintain accurate documentation of client care and progress, while ensuring confidentiality.
- Act as a liaison between clients, families, and caregivers to facilitate communication and enhance the quality of care.
- Participate in ongoing training and professional development to stay current with best practices in home care.
Requirements
- Bachelor's Degree (BSN) and current NYS License or compact License that is in good standing OR associate's degree in nursing with relevant homecare clinical experience.
- Minimum of 1-2 years of Nursing experience with a concentration in the homecare agency space.
- Experience in geriatric care or working with individuals with disabilities.
- Ability to travel within the greater Albany, NY area, as needed.
- Current knowledge of state and federal home health regulations.
- Strong interpersonal and communication skills.
- Proficient in Microsoft Office Suite (Word, Excel, Outlook, etc.).
- Excellent verbal and written communication, organizational, and time management skills.
- Required valid driver’s license and reliable transportation.
Preferred Qualifications
- Bilingual in one of the following languages: Spanish, Russian, Chinese, Korean, or Bengali.
- Ability to work collaboratively with caregivers and families.
- Proficient in electronic health record (EHR) systems.
- Previous experience in a non-skilled home care setting.
- Strong experience using a Uniform Assessment System (UAS).
- Master's degree in nursing.
- Experience in Home Care, Geriatrics, and Case Management; field experience assessing patient care.
- 3+ years of homecare agency experience.

Pay
Base pay for this role is determined within a range and is one part of our total compensation package. The base pay range is between $75.00 and $120.00 per assessment at the commencement of your first year of employment. Compensation decisions depend on factors such as skill set, years of relevant experience, education, location, and licensure/certifications.
