Key Responsibilities

  • Provide primary and preventive care to diverse populations (adult, pediatric, women’s health, geriatric)
  • Perform full history and physical exams, identify gaps in care, and develop treatment plans
  • Order/interprets labs & imaging, request specialist consults, and prescribe medications
  • Conduct visits in homes, nursing facilities, community clinics, and via approved telehealth
  • Collaborate with other NPs and actively participate in team meetings
  • Document care promptly and accurately in EMR

Job requirements

  • Minimum Requirements
  • Master’s degree in Family Health (ANCC/AANP accredited)
  • Active & unrestricted NP license
  • National board certification (ANCC or AANP)
  • Valid driver’s license and willingness to travel
  • EMR proficiency and strong documentation skills
  • Basic Life Support (BLS) certification
  • Diverse patient care population across the lifespan from pediatric to geriatric patients.

Preferred Qualifications

  • 2–5 years as a Nurse Practitioner
  • Home or public health experience
  • Bilingual fluency (preferred)
  • Experience with immunizations and point-of-care testing
