Key Responsibilities
- Provide primary and preventive care to diverse populations (adult, pediatric, women’s health, geriatric)
- Perform full history and physical exams, identify gaps in care, and develop treatment plans
- Order/interprets labs & imaging, request specialist consults, and prescribe medications
- Conduct visits in homes, nursing facilities, community clinics, and via approved telehealth
- Collaborate with other NPs and actively participate in team meetings
- Document care promptly and accurately in EMR
Job requirements
- Minimum Requirements
- Master’s degree in Family Health (ANCC/AANP accredited)
- Active & unrestricted NP license
- National board certification (ANCC or AANP)
- Valid driver’s license and willingness to travel
- EMR proficiency and strong documentation skills
- Basic Life Support (BLS) certification
- Diverse patient care population across the lifespan from pediatric to geriatric patients.
Preferred Qualifications
- 2–5 years as a Nurse Practitioner
- Home or public health experience
- Bilingual fluency (preferred)
- Experience with immunizations and point-of-care testing
