Field Nurse Case Manager

NCA is seeking a Nurse Case Manager in Los Angeles to provide case management services tailored to the special needs and eldercare challenges of clients, focusing on care and quality of life. The role involves collaborating with primary family caregivers, attorneys, financial planners, and trustees.

Responsibilities

• Deliver case management services responsive to client needs, including eldercare and quality of life challenges.
• Collaborate with family caregivers, attorneys, financial planners, and trustees.
• Research and develop solutions to client-specific challenges.
• Meet timely documentation requirements.
• Travel regionally (1-2 times per month overnight) and occasionally nationwide.

Requirements

• Bachelor of Science in Nursing (BSN) required; CCM or CRRN preferred.
• Minimum of 5 years of clinical nursing experience; field case management or workers’ compensation case management experience preferred.
• Experience with third-party benefits, including health insurance, Medicaid, Social Security, and Developmental Disability services.
• Excellent written and verbal communication skills.
• Proficient in MacOS and related software applications (e.g., MS Office).
• Ability to quickly learn and adapt to new technologies in a Mac environment.
• Highly motivated self-starter comfortable working in a virtual office with strong time management and organizational skills.
• Proven ability to collaborate and work through conflicts to achieve goals.
• Strong customer service skills.
