Field Nurse Case Manager

About the role

NCA is in search of a Nurse Case Manager based in San Antonio who will be responsible for case management services responsive to special needs and eldercare client care and quality of life challenges and collaboration with the primary family caregivers, attorneys, financial planners and trustees that are also serving the client.

Responsibilities

• Provide case management services for special needs and eldercare clients.
• Collaborate with primary family caregivers, attorneys, financial planners, and trustees.
• Overnight travel required regionally 1-2 times per month and occasional nationwide travel.
• Meet timely documentation requirements.

Requirements

• Bachelor of Science in Nursing (BSN) required.
• CCM or CRRN preferred.
• Minimum of 5 years clinical nursing experience.
• Experience in field case management or workers’ compensation case management preferred.
• Experience with third‑party benefits: health insurance, Medicaid, Social Security, Developmental Disability services.

Skills

• Excellent written and verbal communication skills.
• Proficient in macOS and related software applications (including MS Office).
• Strong ability to quickly learn and adapt to new technologies and tools in a Mac environment.
• Highly motivated self‑starter with solid time management and organization skills.
• Proven ability to collaborate to accomplish goals and work through conflicts.
• Ability to research and develop solutions to client challenges.
• Excellent customer service skills.
