POSITION SUMMARY
Aid the RN Case Manager in the patient's care plan.
ESSENTIAL FUNCTIONS
- Coordinate with the care team and IDG to implement the patient's care plan.
- Record clinical notes and document pain/symptom management.
- Administer medications and treatments as directed by the physician.
- Document nursing care and interactions with patients and family members.
- Assess patient and family responses and follow up with bereaved families.
- Provide consistent care to all assigned patients, regardless of complexity.
- Participate in on-call rotations.
