About the role
The Field Care Coordinator coordinates person-centered, culturally relevant care plans for individuals receiving long-term services and supports (LTSS).
Responsibilities
- Develop and implement care interventions
- Communicate with stakeholders
- Advocate for patients
- Assess needs and manage social determinants of health
- Conduct home and in-person visits
- Manage member information
- Facilitate referrals to social services
Qualifications
- Current, unrestricted license as an RN or Social Worker
- Experience in community health, behavioral health, and home-based services
- Skills in electronic documentation and MS Office
- Ability to travel within the territory
Preferred qualifications
- Additional certifications
- Experience with managed care or co-occurring disorders
