About the role

The Field Care Coordinator is responsible for assessing and planning care for enrollees, promoting their self-sufficiency and independence. They conduct in-home assessments, develop person-centered care plans, and collaborate with care teams, providers, and community resources to ensure comprehensive, cost-effective care.

Responsibilities

Requirements

• Current license (LPN, Social Work, or related field)
• At least 3 years of care coordination or healthcare experience
• Reliable transportation for travel within the territory

Qualifications

• CCM certification (preferred)
• Experience with Medicaid/Medicare populations (preferred)
• Geriatric or long-term care experience (preferred)
