Field Based HSS Clinical Coordinator RN - Dallas, TX

Responsibilities
- Assess, plan, and implement person-centered strategies
- Connect members with financial, psychosocial, community, and state resources
- Coordinate care across the continuum and serve as a key point of contact
- Advocate for members and families to ensure their needs and preferences are heard and supported
- Collaborate with providers, members, caregivers, and community partners to improve health outcomes
- Travel up to 75% of the time within the Mission, TX area and surrounding areas
Required Qualifications
- Current unrestricted Texas RN license
- 2+ years of experience in a community health or healthcare setting
- Intermediate level of proficiency with Microsoft Office applications
- High-speed internet at residence
- Access to reliable transportation and valid US driver's license with the ability to travel within assigned territory
Preferred Qualifications
- Experience with Medicaid, LTSS, or long-term care services
- Knowledge of HCBS and integrated care principles
- Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations
- Bilingual in English and Spanish
Pay
The hourly pay for this role will range from $29.00 to $52.00 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.

Schedule & Travel
This role requires travel up to 75% of the time within the Mission, TX area and surrounding areas. Remote work flexibility is available for candidates located in the Mission, TX area and surrounding communities within commuting distance. All telecommuters will be required to adhere to UnitedHealth Group's Telecommuter Policy.
