FHW Personal Care Assistant

Responsibilities

• Affords assistance to participants in activity programs at the PACE Center.
• Sets up the PACE center for snacks and lunches.
• Ensures program areas are set up and cleaned daily.
• Communicates any problems or needs in the program to the Center Manager.
• Absolutely reports any accidents, incidents, unsafe and hazardous conditions, and ensures compliance with safety rules.
• Assists with meal and snack times, including feeding as needed.
• Provides personal care such as bathing, toileting, incontinent care, feeding, transfers, lifting, and ambulation.
• Launder participants' clothes and linens after bathing or personal hygiene care.
• Supports ambulation, transfers, and range of motion needs as directed by PACE Physical and/or Occupational Therapists.
• Documents changes in participants' physical or emotional health and communicates this information to relevant staff.
• Coordinates care delivery, obtains participant weights, performs procedures under supervision, and completes documentation as required.
• Participates in daily PCA meetings to discuss personal care services, participants' self-care abilities, social interaction levels, and changes to plans of care.
• Provides escort services for participants to and from home and medical appointments.
• Attends all mandatory in-services and PACE training as required.
• Successfully completes a competency skills test annually for the position.

Qualifications

• Educational Requirement: High school diploma or GED.
• Experience: Some experience as an activity worker, homemaker, personal care aide, home health aide, or nursing assistant preferred. One year of experience working with a frail or elderly population preferred.
• Skills: Discretion, tact, patience, good judgment, empathetic and compassionate attitude in caring for elderly, ability to work within the scope of authority to practice, meet a standardized set of competencies for the specific position described by Fallon Health Weinberg PACE Program, and be medically cleared for communicable diseases and have all immunizations up-to-date.

Pay

$19.00 per hour, reflecting the reasonable and good-faith expectation at the time of posting. Final compensation depends on the candidate's experience, skills, and fit with the role's responsibilities.
