JOB PURPOSE

JOB PURPOSE: As part of the Trueblood settlement, the state is funding enhanced engagement to connect individuals detained in city and county jails with services. This position will provide outreach, engagement, and case management services to individuals who are involved in the criminal court system that have been ordered to undergo a competency evaluation, restoration, or participate in diversion services.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Assist clients to determine which services are needed, such as behavioral health treatment services, housing, food, transportation, medical services, and other social services as identified.
  • Support client's staying connected to services already enrolled in or connect and refer to community behavioral health and substance use treatment.
  • Plans, implements, and coordinates services required to meet the health and human service needs of identified priority clients, including housing, food, clothing, access to primary health care, psychiatric services, and other services as identified.
  • Develop and maintain positive collaborative working relationship with other social services providers, crisis stabilization facilities, crisis responders, evaluation and treatment facility staff, DSHS workers, substance use disorder treatment providers, mental health providers, health care providers, shelter providers, landlords, harm reduction resources, withdrawal management program, protective or representative payees, and other community programs which may support clients.
  • Provide wrap around support to help clients secure and maintain appropriate housing. Meet participants where they are physically and mentally at and engage clients in meeting their treatment goals.
  • Develop trusting relationships with participants, promote a sense of self-direction and self-advocacy, and help the participants plan how they will successfully manage their life.
  • Provides ongoing assessment of a client's cognitive, emotional, physical, social, and vocational strengths and needs.
  • Aids in the development of natural support systems for clients.
  • Advocates for clients with community agencies.
  • Teaches skills such as problem solving, goal setting, money management, symptom management, communication, social skills, and how to access community resources.
  • Uses his or her own lived experience of recovery from addiction/mental illness plus skills learned in formal training to deliver services to eligible clients.
  • Maintains and submits data and reports as required.
  • Maintains familiarity with and handles client information in accordance with Federal Regulations (42 CFR, Part 2), the Revised Code of Washington (RCW 71.05.390 and RCW 71.24), and other applicable laws pertaining to confidentiality of client and staff information.
  • Maintains physical security of confidential materials and assigned Agency property.

SECONDARY DUTIES AND RESPONSIBILITIES

  • Performs relevant clinical and administrative tasks according to agency policies.
  • Demonstrates proficiency in utilization of the Agency's electronic medical record system.
  • Participate in team meetings and program development.
  • Provides information and education to community members.
  • Maintains high standards of professional conduct in interaction with clients, staff, and other community members.
  • Maintains professional competence through a program of professional development and in-service training.
  • Demonstrates flexibility in adapting to changing work demands.
  • Performs other duties as assigned or requested.

  • Understands Peer Support ethical and practice guidelines of being open minded, recovery oriented, hopeful, respectful, empathetic agent of change, strengths focused, and person centered.

KNOWLEDGE, SKILLS, AND ABILITIES

  • Essential: Ability to identify client capabilities regarding education, work, and leisure activities.
  • Able to multitask and effectively manage a caseload of individuals with diverse needs.
  • Able to identify individual coping strategies and supports.
  • Able to elicit client choices.
  • Able to use sound judgments in intervention, support services, and problem solving.
  • Able to educate clients about signs, symptoms, and recovery from mental illness and/or substance use disorders.
  • Knowledge of social service delivery systems, particularly as related to homeless adults.
  • Able to coordinate care with multiple service providers.
  • Able to manage crises effectively to ensure safety.
  • Able and willing to work independently and as a team member.
  • Able to multitask and effectively manage a caseload of individuals with diverse needs.
  • Strong knowledge of community resources.
  • Must have competence using computer, photocopier, fax machine, smartphone and entering data into electronic database.
  • Secondary: Able to help clients transition from incarceration or hospitalization to clinical services and natural support.
  • Able to advocate effectively with community partners.
  • Basic knowledge of psychotropic medication and its possible side effects.
