Responsibilities
- Establishes a trusting relationship with at-risk families by providing regularly scheduled home visits from intake through enrollment up to 5 years for each family as determined by HFA Accreditation Standards.
- Participates in weekly mandatory supervision, with a focus on administration, Clinical and reflective strategies.
- Ensure accurate and timely reporting and case documentation in written or electronic Healthy Families NY database and management information systems, within the required time frames.
- Maintain at least a 75% home visit rate as determined by HFA Best Practice Standards and maintain a case load of up to 25 families.
- Provide continuous outreach to families and community referral sources to maintain caseloads and program capacity as determined by Office of Children and Family Services NYS contract.
- NYS mandated reporters and must follow HFNY policies and procedures related to the NYS law for mandated reporters which includes mandatory annual training.
- Functions as a representation of HFRC/SPHP at community wide events and works collaboratively with community agencies.
Requirements
- HSD/GED Required.
- Must be Fluent in Spanish required.
- Relevant experience working with children and families.
- A valid NYS driver's license required.
- Valid automobile insurance is required.
- Can potentially travel up to 400 miles a month in both rural and urban communities and are reimbursed for mileage by the set federal reimbursement rate.

- Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.

Pay
- Pay Range: $17.85 - $22.20
- Pay is based on experience, skills, and education.
- Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
