Family Support Specialist - Eagle House

About the role

The Family Support Specialist at Eagle House Program provides direct services to children and their families, focusing on comprehensive assessments, treatment plan development, implementation, discharge planning, and case management. This role also involves developing and maintaining collaborative relationships with other community agencies and organizations.

Responsibilities

• Conducts comprehensive assessments and engagement
• Develops treatment plans and contracts for services
• Implements treatment plans, modifying as needed
• Discharge planning, including follow-up treatment and community services connections
• Provides case management services
• Works with community groups in collaborative ventures
• Completes clinical documentation and reports accurately and timely
• Maintains collaborative relationships with other community agencies and organizations
• Represents The Village in community groups or committees
• Participates in agency-wide task forces, committees, or problem-solving teams
• Contributes to agency organizational life and visibility

Requirements

• Bachelor’s degree in social work, sociology, psychology, or a comparable human service program
• 2-3 years’ experience in a comparable position in a human services agency (preferred but not required)
• Excellent oral and written communication skills, comfortable with children, parents, and school personnel
• Able to work independently with minimal supervision, flexible, and multitask capable
• Cultural competency to serve targeted populations, bilingual skills preferred
• Experience working with inner-city children
• Commitment to results/performance based accountability
• Valid driver's license and ability to drive or obtain transportation to provide services throughout the service area
• Proficient with Microsoft Office and spreadsheet applications
• Familiarity with specific applications used by The Village a plus
