Family Service Coordinator Bilingual

About the role

Family Service Coordinators provide case management/service coordination for children ages zero to three who evidence a developmental delay or other atypical development.

Responsibilities

• Conduct screenings
• Facilitate eligibility determination
• Organize and participate in evaluations
• Develop plans for care
• Link families with community resources
• Teach families how to be effective advocates for their children
• Assist families in arranging for services their children may need at the time of discharge

Qualifications

• Knowledge of the local service delivery system, the target group’s needs, the need for integrated services, and the resources available or needing to be developed
• A Bachelor’s Degree in a human service related field (Human service related fields are those that include coursework in areas such as human development, psychology, social services or similar areas)
• Three years or more of experience as a Family Service Coordinator (or equivalent case management position) preferred
• One (1) year of supervised experience working with families with special needs
• A valid Wisconsin driver’s license, reliable transportation and car insurance are required
• Bilingual in Spanish required
