About the role
Family Service Coordinators provide case management/service coordination for children ages zero to three who evidence a developmental delay or other atypical development.
Responsibilities
- Conduct screenings
- Facilitate eligibility determination
- Organize and participate in evaluations
- Develop plans for care
- Link families with community resources
- Teach families how to be effective advocates for their children
- Assist families in arranging for services their children may need at the time of discharge
Qualifications
- Knowledge of the local service delivery system, the target group’s needs, the need for integrated services, and the resources available or needing to be developed
- A Bachelor’s Degree in a human service related field (Human service related fields are those that include coursework in areas such as human development, psychology, social services or similar areas)
- Three years or more of experience as a Family Service Coordinator (or equivalent case management position) preferred
- One (1) year of supervised experience working with families with special needs
- A valid Wisconsin driver’s license, reliable transportation and car insurance are required
- Bilingual in Spanish required
