About the role
As a Primary Care Physician within our PACE program, you will work closely with an interdisciplinary team to coordinate and deliver individualized care plans for seniors with complex medical and social needs. This model allows you to build deeper relationships with patients and directly see the impact of your care.
Responsibilities
- Lead collaborative care planning within the interdisciplinary team (IDT)
- Cook up transitions of care, including hospital discharges and long-term placement planning
- Provide preventive, chronic, and acute care management for geriatric patients
- Participate in a rotating phone-only call schedule approximately once per month
- Collaborate with Nurse Practitioners, Physician Assistants, specialists, and care teams to optimize patient outcomes
Qualifications
- Completion of an ACGME-accredited residency program
- Active and unrestricted physician license
- DEA registration and CDS certification
- Eligible for Medicare enrollment
- 2+ years of primary care experience preferred
- Geriatric fellowship experience highly valued
- Experience working within an interdisciplinary team structure preferred

Pay
$275,000 - $330,000+ based on experience
Schedule
Physicians primarily work onsite at the center, with one day typically dedicated to home visits and visits to senior living facilities.
