What You Will Do To Change Lives
- Engage family members and prospective participants to foster willingness to accept available services
- Provide information, education, and resources to family members to enable them to better support their loved one in their goals and/or recovery plan, whether living independently or in the family home
- Provide direct support and case management services to assigned family members and participants
- Work closely with participants and their families to ensure their needs are expressed and that they have a "voice" in their recovery plan
- Complete program intakes/assessments in conjunction with the clinical team
- Facilitate groups and promote engagement and provision of peer and family support services
- Serve as an advocate for the participant and family for services as a point of interaction between participant's family and the program
Shifts Available
Full-Time | DAYS | Shifts: 8:30 AM - 5:00 PM | Days: Monday - Friday
What You Bring To The Table (Must Have)
- Bachelor's Degree or at least two (2) years' experience in a mental health services environment
- Peer Certification must be obtained prior to hire date and maintained throughout employment
- California: Medi-Cal Peer Support Specialist Certification through CalMHSA must be obtained within 120 days of your date of hire and maintained throughout employment. Failure to obtain certification within this period will result in separation with the company.
- At least five (5) years of demonstrated experience as a parent or primary caregiver of a person with behavioral health care needs
- Valid and current driver's license, and personal vehicle insurance with your name listed as a driver
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.

Sign-On Bonus: $1,500.00
Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

About the program: Visions is a Full Service Partnership Intensive Case Management (FSP-ICM) program serving up to 191 adults with serious mental illness who require a higher level of community-based behavioral health support to achieve stability, recovery, and independence. The program utilizes a multidisciplinary team approach and operates under a "whatever it takes" philosophy, providing individualized, recovery-oriented services tailored to each participant's unique needs and goals.
