Family Nurse Practitioner (FNP)

Full-Time, On-site position at an outpatient clinic in West Springfield, MA.

About the Role

The Center for Human Development (CHD) is seeking a Family Nurse Practitioner for a brand-new role designed intentionally around provider sustainability and clinical quality. You will collaborate with leadership to shape the structure of this position, ensuring it works for both you and the community.

Responsibilities

As a trusted Family Nurse Practitioner, you’ll provide comprehensive primary care across the lifespan, including:

• Chronic disease management
• Preventative education
• Evidence-based medicine within a supportive, multidisciplinary regional care team

Requirements

• Master’s or Doctoral degree with FNP Certification
• Valid MA APRN license
• Strong clinical judgment and a passion for community-based, patient-centered care
• Open to both experienced APRNs seeking long-term sustainability and high-achieving new grads with top-tier clinical training
