About the role
We build long-term relationships with our patients, managing everything from preventive care and chronic disease management to acute illnesses and minor procedures.
Responsibilities
- Work closely with nurse practitioners, physician assistants, nurses, and care coordinators to ensure seamless, coordinated care.
- Offer lab, imaging, and access to specialty care in the adjacent hospital, allowing for efficient diagnosis and treatment.
- Ambitious to embrace technology and innovation, including telehealth, digital health tools, and population health strategies to improve outcomes and access.
- Deeply rooted in the communities we serve, with a strong emphasis on health education, outreach, and preventive care.
- Join 2 FM, 1 PA, 1 Pediatrician
- Phone-only call rotation (1:8) with nurse triage and no hospital call
- Practice at one location with clinic hours 8:30-5pm M-F
- Three-year salary guarantee moving to production-based compensation
- Starting bonus
- Up to $15,000 moving allowance
