Position Overview

Community Memorial is seeking a Board-Certified or Board-Eligible Family Medicine Physician to join a well-established primary care practice in the picturesque community of Hamilton, New York. Key Responsibilities

  • Provide full-spectrum family medicine services in an outpatient setting.
  • Diagnose and treat acute illnesses, injuries, and chronic medical conditions using evidence-based medical practices.
  • Develop individualized treatment plans tailored to each patient's medical history, lifestyle, and healthcare goals.
  • Conduct annual wellness visits, routine physicals, preventive screenings, and immunizations.
  • Perform comprehensive patient evaluations and clinical assessments.
  • Manage chronic conditions through ongoing monitoring, medication management, and patient education.
  • Coordinate care plans designed to improve long-term patient outcomes and quality of life.
  • Monitor treatment effectiveness and adjust care plans as needed.
  • Order and interpret laboratory studies, diagnostic testing, and screening exams.
  • Review diagnostic findings and consultant recommendations to ensure accurate diagnosis and treatment.
  • Promote preventive healthcare through education, counseling, and risk-factor reduction strategies.
  • Collaborate with specialists, hospitals, therapists, behavioral health providers, and other healthcare professionals.
  • Facilitate referrals and ensure continuity of care across the healthcare continuum.
  • Work closely with advanced practice providers, nursing staff, care managers, and support personnel to provide exceptional patient care.
  • Maintain accurate and timely documentation within the electronic medical record (EMR).
  • Participate in quality improvement, patient safety, and population health initiatives.
  • Ensure compliance with organizational policies, regulatory requirements, and clinical best practices.

Qualifications

  • MD or DO degree from an accredited medical school.
  • Board Certified or Board Eligible in Family Medicine.
  • Eligible for or currently holding an unrestricted New York State Medical License.
  • Current BLS, ACLS, and PALS certifications preferred.
  • Strong clinical judgment and commitment to evidence-based medicine.
  • Excellent communication and patient relationship-building skills.
  • Commitment to delivering compassionate, patient-centered care.

Compensation & Benefits

  • Base Salary: $250,000 - $290,000
