Responsibilities
- Provide outpatient care with nurse triage as the first point of contact
- Participate in call duties by phone, averaging less than one week per year
- Collaborate with a multidisciplinary team, including MTM Pharmacists, Care Coordination, Nurse Call Center, Diabetes Educators, Community Paramedics, and Social Workers
Qualifications
- Graduate of an accredited Medical or Osteopathic School
- Completion of an accredited ABMS-approved Family Medicine or Internal Medicine residency
- Ability to obtain a Minnesota Medical Doctor or Doctor of Osteopathy license
- Valid DEA Registration
