Family Medicine Care Coordinator I
About the role
The Family Medicine Care Coordinator works within a multi-disciplinary health care team in the delivery of comprehensive services for patients diagnosed with chronic illness and high risk pediatric patients in a primary care setting. Acting as the primary contact person for the family, the Family Medicine Care Coordinator assists the health care team in the development and implementation of a health care plan tailored to the needs of the client and the client's family in order to promote continuity of care, improves referral tracking, and ultimately reduces the rate and severity of diabetes-related complications.
Responsibilities
- Assists the health care team in the development and implementation of a health care plan tailored to the needs of the client and the client's family.
- Works independently and as part of a team to support the health care team in managing chronic illness and high-risk pediatric patients.
- Coordinates patient care plans and ensures effective communication between the patient, family, and healthcare providers.
- Facilitates referrals and coordinates services to ensure continuity of care.
- Improves referral tracking and helps reduce the rate and severity of diabetes-related complications.
Qualifications
- Must have a Bachelor's degree in Health Education/Health related field or hold California state licensure for vocational nursing (LVN).
- This position is considered an entry-level position, but experience in practical nursing, preferably in an ambulatory care facility or a health education background in chronic disease is strongly desirable.
- Bilingual in English and Spanish required, with excellent written and oral communication skills.
- Possess the ability to work independently and as a team member.
- Cultural competency required.
- Excellent organizational and problem-solving skills and the ability to prioritize are essential.
- Willingness to travel to provide outreach presentations.
- Have current CA Driver's License, available vehicle, and valid liability insurance.
- Experience with basic computer skills including word processing and data entry.
- Ability to perform a variety of tasks after training.
