About the role
Family or Internal Medicine Physician, with a focus on Geriatrics, providing comprehensive care to Veterans in a Community Living Center.
Responsibilities
- Reviewing patient histories, examinations, assessments, and care plans for CLC residents.
- Coordinating day-to-day management of Veteran care with interdisciplinary teams.
- Facilitating hospital admissions, post-discharge care, and specialty consultations.
- Participating in screening, consults, and telephonic urgent care coverage.
- Engaging in clinical team meetings and other assigned duties.
Qualifications
- Board Certified or Eligible in Family Medicine, Internal Medicine, or Geriatrics.
- Unrestricted license from any US State or territory.
Skills
- Experience working with multidisciplinary teams and geriatric populations preferred.
