Family Coach/Case Manager - Bilingual in Spanish
About the role
We are seeking a Bilingual Family Coach/Case Manager to join our Family Hope Center. As a Family Coach, you will assess individual clients and families for their physical, social, emotional, and spiritual needs. You will provide family coaching and case management services to assist with issues such as poverty, drug addiction, child and spousal abuse, and homelessness. You will organize support groups, help clients obtain community services, refer patients to other agencies, and collaborate with civic, religious, and business groups to address social problems through community programs.
Spanish language proficiency is required, including the ability to communicate effectively verbally and in writing, to serve Spanish-speaking clients.
Responsibilities
- Create, develop, and implement Family Hope Center programs aligned with approved core programs and the Family Strengthening Framework to meet the physical, emotional, social, and spiritual needs of individuals and families as outlined in each person’s service plan.
- Assist with the design and coordination of supportive services for families in accordance with program goals and objectives.
- Assist eligible program participants with out-of-home placement or respite care when necessary.
- Compile and analyze intake and referral information to assess clients’ and families’ needs; complete the enrollment process and all necessary paperwork accurately and timely.
- Provide case management services, including reviewing, evaluating, and updating individualized service plans, case notes, financial distributions, and referrals.
- Coordinate services for group and individual therapy for assigned families and clients, and provide crisis intervention counseling as appropriate.
- Assist program participants with accessing community resources, including program enrollments and other services.
- Develop professional relationships with institutions and agencies providing education, job training, living skills training, employment services, housing, and child care to coordinate services for program participants.
- Analyze program needs; develop, modify, and implement changes to meet the evolving needs of clients.
- Create and manage schedules for events and classes; coordinate and facilitate meetings and trainings for clients and groups, including Substance Abuse, Parent Education, Conflict Resolution, and Anger Management.
Requirements
- Bachelor’s Degree in Social Work or a related field.
- Spanish language proficiency, including effective verbal and written communication skills.
- Minimum of 2 years of prior case management experience working with at-risk children and families.
- Proficient knowledge of and/or work experience using community resources.
- Ability to drive assigned or personal vehicle with a valid state license, proof of liability insurance, and eligibility to be insured under Buckner’s policy; must be at least 21 years old to drive on behalf of Buckner.
- Ability to use up to 50 pounds of force occasionally, and/or up to 20 pounds frequently, and/or up to 10 pounds continuously to move objects and/or people.
- Ability to walk, stand, and sit, sometimes for prolonged periods.
