About the role
Deliver high-quality psychotherapeutic services to children, adolescents, and families using the Family Centered Treatment (FCT) model.
Responsibilities
- Provide FCT clinical services in clients' homes and communities, ensuring model fidelity and completing required documentation.
- Develop and coordinate service plans with Treatment Teams, conduct assessments, and facilitate family engagement through psychoeducation and intervention.
- Offer 24/7 on-call support, assist families with goal setting, discharge planning, and accessing community resources.
- Collaborate with medical and non-medical providers to ensure coordinated care.
Requirements
- Master's Degree in Social Work, Counseling, or related field.
- At least two years of experience working with youth or families.
- FCT certification preferred; must complete certification within one year if not already certified.
- State-specific licensure or credentialing requirements.
