About the role
Seeking an Advanced Practice Provider to deliver outpatient primary care at a Family Care Clinic in Prineville, OR.
Responsibilities
- Evaluate, diagnose, treat, and document patient encounters across all ages
- Care for patients in a Medical Home model, coordinating services with behavioral health, pharmacy, and care management
- Develop comprehensive treatment plans incorporating various specialties
- Follow evidence-based family medicine practices
- Prescribe medications
- Interpret diagnostics
- Provide patient education
- Maintain documentation per standards
Requirements
- A Master’s in Physician Assistant Studies or Nursing
- Valid licensure
- Certifications
- Outpatient care experience
Qualifications
- 2+ years in family medicine
- Familiarity with Medical Home models
- Epic EHR experience preferred

Pay
Compensation ranges from $154,568 to $165,351 annually
Schedule
Work at least 40 hours weekly with scheduled patient contact, participate in call rotations, and collaborate with a multidisciplinary team.
Location
Relocation to Prineville is required.
