Family Care APP (NP or PA)
Overview
The Advanced Practice Provider (APP) provides outpatient medical care across all ages under supervision in a Family Medicine Medical Home setting.
Responsibilities include evaluating, diagnosing, treating, and documenting patient encounters, coordinating with multidisciplinary teams to ensure cost-effective, personalized care. They develop treatment plans incorporating behavioral health, pharmacy, therapy, nutrition, and surgery, monitor ongoing care, and refer to specialists as needed.
Work is based at the Prineville Clinic, with a minimum of 40 hours/week, including participation in shared call rotations evenings, weekends, and holidays.
Requirements
- Master’s in Physician Assistant Studies or Nursing
- Valid licensure
- Certifications
- Outpatient experience
Preferred Qualifications
- Doctorate
- Oregon licensure
- 2+ years' family practice experience

Schedule
