Family Care APP (NP or PA)

Overview

The Advanced Practice Provider (APP) provides comprehensive outpatient medical care across all ages under supervision, including evaluation, diagnosis, treatment, and documentation. They coordinate services within a Medical Home model, developing treatment plans that incorporate behavioral health, pharmacy, nursing, therapy, nutrition, or surgery, and monitor patient progress with referrals as needed.

Work Location and Hours

Work is based at the Prineville Clinic, with a minimum of 40 hours/week, including scheduled call rotations covering evenings, weekends, and holidays, supported by backup physicians.

• Delivering evidence-based family medicine

Qualifications

• A Master’s in Physician Assistant Studies or Nursing
• Licensure
• Certifications
• Outpatient experience
• Teamwork skills

Preferred Qualifications

• 2+ years’ experience
• Familiarity with Medical Home models
• Epic EHR proficiency
