Family Advocate

About the Role

This position involves supporting families of children or youth with special health care needs (CYSHCN) and disabilities by leveraging regional knowledge of lifespan disability services and systems.

Responsibilities

• Utilize knowledge of lifespan disability services and supports across the region.
• Participate effectively in multidisciplinary medical clinics.
• Assist families in navigating service delivery systems, including medical, school, transition (up to age 21), and health and welfare services.
• Provide peer-to-peer mentoring to support families of CYSHCN and disabilities.

Requirements

• Personal experience with navigating disability service systems.
• Background in multidisciplinary medical and service programs.
• Strong communication and mentoring abilities.

Qualifications

Preference for candidates who are parents or family members of CYSHCN or children with disabilities.
