About the Role

The Advanced Practice Provider (APP) practices in a clinic as part of a collaborative team that may include Family Practice and/or Internal Medicine Physicians, RN Case Managers, and clinic staff. This team provides a comprehensive, coordinated model of care. The APP primarily manages the patient population either in the presence of, or in conjunction with, the primary care physician and/or specialist. The APP may see patients in either the outpatient and/or inpatient setting, depending on the group's needs. The role acts as an extension of the primary care physician to provide both chronic and acute care. Performance will be tracked and reported using metrics related to the organization's strategic direction and operational needs.

Responsibilities

  • Provide care consistent with licensure and/or certification.
  • Diagnose and treat both acute and chronic conditions such as diabetes, hypertension, kidney disease, and other chronic medical conditions.
  • Treat acute conditions such as minor injuries and other acute medical conditions like infections or exacerbations of chronic diseases.
  • Order and interpret diagnostic lab work, x-rays, and EKGs.
  • Prescribe medications and other treatment modalities as needed.
  • Counsel patients on disease conditions to ensure they understand their disease process, treatment, and expected outcomes.
  • Work as an integral part of a comprehensive and collaborative team to provide quality evidence-based care.
  • Perform all other related duties as assigned.

Requirements

  • Graduate credit from a PA/NP Program.
  • Current licensure through the Georgia Board.
  • Current prescriptive authority from the Georgia Board.
  • Current BLS certification from the American Heart Association.
  • 2+ years of primary care experience.
