Avenues Home Care serves the Nashville area, including Nashville, Belle Meade, Green Hills, Franklin, Goodlettsville, Hendersonville, Gallatin, Lebanon, Mount Juliet, Murfreesboro, Smyrna, Ashland City, Hermitage, and many other nearby communities. We take the time to match clients with the right caregiver through our CareMatch program, using behavioral assessment tools to ensure the best possible care.
Responsibilities
- Assist clients with Activities of Daily Living (ADLs) as specified in the client’s plan of care, including but not limited to:
- Bathing, dressing, grooming, and other personal hygiene
- Toileting, transferring, eating, and continence care
- Provide medication assistance, housekeeping, laundry, meal preparation, feeding, shopping, and transportation to appointments
- Offer companionship and perform other household tasks as outlined in the client’s plan of care
- Report daily activity to your supervisor through the agency management system, daily logs, and other communication methods (phone, email, agency app, or in-person)
- Practice safety in all client-related activities
- Lift or push up to 50 pounds as required
- Demonstrate honesty, accountability, and integrity in all interactions
- Provide superior care and remain vigilant of any changes in the client’s condition
- Maintain a professional appearance in compliance with Avenues Home Care policies, including dress code
- Report for shifts on time and communicate scheduling changes promptly
- Report any suspected abuse or neglect immediately, following legal requirements
- Document unusual behaviors, events, conditions, or individuals
- Maintain confidentiality of all records and correspondence
- Attend meetings and in-service training as requested
- Abide by all company policies and procedures, including HIPAA regulations
- Perform other duties as assigned
Requirements
- Strong communication skills and ability to follow verbal and written instructions
- Proficiency with the Homecare Company Management System (ClearCare)
- Ability to maintain an established work schedule
- Effective interpersonal, organizational, and planning skills with attention to detail
- Ability to establish and maintain effective working relationships
- Must be “Fit for Duty” and possess:
- Valid driver’s license and automobile insurance
- 18 years of age or older
- CNA or PCA license (required)
- CPR and First Aid certifications
- Negative TB test
- Reliable transportation
Preferred Qualifications
- Prior experience assisting with ADLs or caring for elderly, disabled, or vulnerable clients
Skills
- Interpersonal and communication skills, including tact and diplomacy
- Confidentiality and adherence to HIPAA regulations
- Use of telephone, smartphone, or tablet for reporting and communication
- Proper use of personal protective equipment (PPE)
Schedule
- Full-time and part-time positions available
- 8-hour, 10-hour, and 12-hour shifts
- PRN and short shifts
