Avenues Home Care of Tennessee serves the Northeast Tennessee area including Bristol, Johnson City, Bluff City, Blountville, Piney Flats, Sturgeon, Oak Grove, and surrounding communities. Our CareMatch program ensures clients are paired with the right caregiver through behavioral assessments and personalized care plans.
About the role
As an Experienced Caregiver, you will provide compassionate care to elderly, disabled, and vulnerable clients who cannot care for themselves. Your primary focus will be meeting the client’s needs as outlined in their individualized plan of care, while upholding the high standards of the Avenues Home Care Way.
Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, transferring, eating, and continence care.
- Provide medication assistance, light housekeeping, laundry, meal preparation, feeding, shopping, and transportation to appointments.
- Offer companionship and perform other household tasks as specified in the client’s plan of care.
- Report daily activity to your supervisor through the agency management system, daily logs, and other communication methods (phone, email, app, or in-person).
- Practice safety in all client-related activities and lift or push up to 50 pounds as required.
- Demonstrate honesty, accountability, and integrity in all interactions.
- Remain vigilant for changes in client condition or potential care concerns and report them immediately.
- Maintain a professional appearance in compliance with company dress code and policies.
- Report suspected abuse or neglect through the proper chain of command, as required by law.
- Document unusual behaviors, events, conditions, or individuals.
- Maintain confidentiality of client records, correspondence, and sensitive information in accordance with HIPAA regulations.
- Attend meetings and in-service training as requested.
- Adhere to all company policies and procedures.
- Perform other duties as assigned.
Requirements
- Strong communication skills and the ability to follow verbal and written instructions.
- Proficiency with the Homecare Company Management System (ClearCare).
- Ability to perform essential job functions with or without reasonable accommodations.
- Must be 18 years or older with a valid driver’s license, reliable transportation, and automobile insurance.
- Must maintain an established work schedule and be physically fit for duty.
- Effective interpersonal, organizational, and planning skills with attention to detail.
- Ability to establish and maintain effective working relationships while maintaining confidentiality.
Qualifications
- CNA License or prior experience as a caregiver (preferred).
- CPR and First Aid certifications (preferred).
- Prior experience assisting with ADLs or caring for elderly, disabled, or vulnerable clients.
Schedule

- 8, 10, or 12-hour shifts
