Evening caregiver

About the Role

Community Horizons is seeking caregivers to provide one-on-one, in-home care for a young female member in her home setting. This position is ideal for someone who enjoys building meaningful connections and providing hands-on support with daily care needs in a comfortable home environment with family support.

The member enjoys puzzles, Scrabble, horses, and engaging in activities. We are looking for a kind, attentive, and personable caregiver who can create a positive and respectful environment.

Responsibilities

• Provide full-care assistance, including use of a Hoyer lift, help with showers, dressing, brief changes, and transfers.
• Assist with daily living activities such as moving patients in and out of beds, baths, wheelchairs, or automobiles.
• Bathe patients and assist with personal care, including dressing and grooming.
• Care for patients by changing bed linens, washing and ironing laundry, and cleaning.
• Engage with the member through conversation, reading aloud, or activities to keep them mentally healthy and alert.
• Plan, purchase, prepare, or serve meals according to prescribed diets.

Schedule

Hours are approximately 4 PM – 9 PM on a two-week rotating schedule.
