About the role

The PACE LIFE Program Escort is accompanying participants in door-to-door transportation to the PACE Center, home, medical appointments or other destinations as part of the PACE transportation services. The Escort also assumes responsibility for communication while the van is in motion and reporting any changes observed in the patient's condition to appropriate clinical personnel. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to team work and cooperation.

Responsibilities

• Accompany participants in door-to-door transportation to the PACE Center, home, medical appointments or other destinations as part of the PACE transportation services.
• Assume responsibility for communication while the van is in motion and report any changes observed in the patient's condition to appropriate clinical personnel.
• Support organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to team work and cooperation.

Qualifications

• High school diploma or equivalent required.
• Ability to read, write and speak English required. Bilingual preferred.
• BLSHCP required within 60 days from date of hire or transfer.
• Minimum 1 year working with the frail or elderly required.
