ER Discharge Coordinator

About the role
As an ER Discharge Coordinator, you will be responsible for Point of Care (PoC) Patient Bedside scheduling as part of the company’s centralized ER and Urgent Care Follow-up programs. The role focuses on a bedside-initiated patient experience to increase contact rate, scheduling immediacy, and improve overall patient experience while maintaining a positive customer experience.
Responsibilities
- Speak with customers, patients, and stakeholders in a pleasant and professional manner.
- Communicate appropriately and clearly with department leadership, co-workers, and customers/patients.
- Schedule patients for follow-up care bedside when appropriate.
- Ensure performance and quality standards are met.
- Provide superior customer service to internal and external customers and patients.
- Support the vision and strategy of the organization, positively promote services offered, and escalate issues to leadership as needed.
- Suggest ideas for positive changes to department policies and procedures.
Requirements
- One year of related experience is preferred.
