Entitlements Specialist

About the role

RiseWell Community Services is seeking a Part Time Benefits Specialist to work across the ACT Teams in Babylon & Medford, NY. This is a part time position/ 21 hours per week.

Responsibilities

• Ensure clients have active health insurance and income source
• Participate in a shared caseload of the clients enrolled on the ACT/FACT program roster
• Provide support based on program and agency need

Requirements

• High School Diploma or GED
• Valid and clean NY State driver license
• Fingerprinting, criminal record check, and approval from NYS Office of Mental Health required
