Evaluations and Progress Monitoring

  • Evaluates skills and qualifications
  • Advise on career paths
  • Promote employment opportunities
  • Maintain ongoing relationship with each business contact
  • Monitor, document, and evaluate the progress of clients

Crisis Intervention and Referrals

  • Provide crisis intervention as necessary
  • Facilitate the transition of appropriate referrals
  • Ensure adequate assessment of individuals
  • Communicate regularly with Employment department representatives

Job Retention and Advancement

  • Work with individuals on job retention and advancement

Interpersonal and Communication Skills

  • Excellent interpersonal and communication skills
  • Help develop motivation, skills, and de-escalation tactics

Document Management

  • Write ISP's, Monthly reports, SalesForce and HCSIS documents for the employment program
  • Keep case files (hard copy and electronic) up to date

Other Duties

  • Perform all other duties as assigned

Qualifications

  • Exercise independent professional judgment and strong decision-making skills
  • Strong communication skills with critical conflict resolution skills and de-escalation techniques
  • Provide on-call assistance to individuals
  • Strong organizational skills

Education and Experience

  • Associates Degree
  • Experience with persons with disabilities

Physical Requirements

  • Navigate public transportation
  • Must be able to lift up to 15 pounds at times
