Eligibility RN

Responsibilities

• Schedules and conducts home visits to make level of care assessments for participant eligibility.
• Completes respective documentation in collaboration with the InnovAge Primary Care Physician and compiles supporting documentation for enrollment.
• Participates and assists with outreach programs.
• Serves as a member of the Interdisciplinary Team (IDT) and coordinates appointments for prospects requiring further assessments by other IDT members.
• Participates in the case management of peer reviews of client records and provides consultation to case managers and other staff members regarding medical and nursing issues.
• Maintain knowledge and expertise in eligibility, enrollment, and program specifications for Medicare and Medicaid, specifically as they apply to the Program of All-Inclusive Care for the Elderly (PACE).
• Assists in completing in-home assessments, reassessments, supervisory visits, and routine RN care as scheduled and needed by the department.

Requirements

• Associate degree from an accredited nursing school.
• Current Registered Nurse license in the state where working.
• Current First Aid and BLS certifications required prior to hire. Acceptable vendors are from either American Heart Association and/or American Red Cross.
• 3+ years of health care experience with a focus on geriatrics.
• 1+ year of experience working with the frail or elderly.

Preferred

• Bachelor’s degree in nursing.
• Certification as a Gerontological Nurse.

Compensation

The pay range for this position is $38.80 - $50.48 per hour, and it may vary based on job-related factors such as work location, experience, knowledge, skills, education, certifications, training, and internal equity.
