About the role
The Supported Living Program Case Manager is responsible for providing comprehensive support to individuals with disabilities in their homes.
Responsibilities
- Assess and document individual needs and preferences
- Develop and implement care plans
- Coordinate services and resources
- Maintain regular communication with clients and families
- Provide crisis intervention and support
- Ensure compliance with regulations and standards
Requirements
- Bachelor’s degree in Social Work, Psychology, or related field
- At least 2 years of relevant experience in disability services
- Valid driver’s license and access to reliable transportation
- Ability to work flexible hours including evenings and weekends
Qualifications
- Strong interpersonal and communication skills
- Proficiency in Microsoft Office Suite
- Knowledge of community resources and services
Skills
- Excellent organizational and time management skills
- Ability to handle sensitive and confidential information
- Experience with case management software
