Policies

• Criminal Background Check: Futures Unlimited, Inc. conducts Criminal Background Checks on all staff. Individuals convicted of certain felonies (as specified in the Illinois Health Care Worker Background Check and Agency Policies Act) are prohibited from being employed in a Direct Care position unless a waiver is granted by the Illinois Department of Human Services.
• Nurse Aid Registry Check: Staff must pass a check through the Illinois Department of Public Health Nurse Aid Registry before being hired.
• Pre-Employment Drug Test: Staff will undergo a Pre-Employment Drug Test. Employment is contingent upon the results of this test. If the results are positive, employment will not be granted.
• Physical Function Test: Some positions may also require a Physical Function test.
