About the Organization

Citizen Care Inc., a subsidiary of Partners For Quality Inc., provides residential, day program, habilitation, supported employment opportunities and behavioral support services to adults with intellectual and developmental disabilities. We are a non-profit organization, accredited by the Council on Quality & Leadership. Citizen Care has locations throughout the West Hills and South Hills of Allegheny County.

Schedule & Location

  • Schedule: Monday through Friday, 8:00 AM - 4:00 PM
  • Hours: 40 hours per week
  • Location: Walden Senior Citizens Center

Responsibilities

  • Treating people with dignity and respect
  • Encouraging personal discovery, development, and enhancing self-esteem
  • Promoting independence in all aspects of a person's life
  • Encouraging each person to understand, exercise, and protect their rights and the rights of others
  • Helping each person to identify, realize and celebrate their hopes and dreams
  • Encouraging independence while at the same time providing supports as needed
  • Acting as a community ambassador representing the agency in a positive manner by demonstrating professionalism when with the general public/business contacts
  • Identifies, pursues, schedules, and coordinates opportunities for individuals to develop and retain relationships between program clients and community members
  • Helping to nurture volunteer opportunities by establishing contacts and remaining an active confidant during the experience(s)
  • Accompanying and assisting people during activities/trips up to 75% of the time
  • Interacting positively with people at all times
  • Problem solves and assists the program to overcome barriers and challenges within the community
  • Provides behavioral supports including execution of individual plans, crisis plans, SEEP, TSP's, etc.
  • Ensures the safety and welfare needs of the individuals during community inclusion
  • Complies with HIPAA regulations
  • Basic computer knowledge of e-mails, Word, Excel, MS Outlook
  • Assisting people to achieve/realize personal outcomes/personal goals including care
  • Assisting people with their health and dietary needs
  • Assisting people with safety and security
  • Accurate paperwork and record keeping, including but not limited to completion of all components of each individual's Plan for Services and Supports
  • Assisting in the overall operations of the Community Participation Supports by providing a safe, healthy, and productive environment that nurtures success and fosters a rewarding experience for all people receiving services and employees

Qualifications

  • High School Diploma / GED
  • Must be 18 years of age or older
  • Reliable means of transportation
  • Driver's license
  • One (1) year work experience providing services and supports to people with intellectual disabilities
  • The ability to lift and transfer without restrictions
  • Act 34 and if applicable, FBI clearances
