Responsibilities
- Provide support, counseling, and case management services
- Coordinate care plans and collaborate with treatment teams
- Conduct assessments related to mental health and substance use needs
- Maintain clinical documentation
Requirements
- Bachelor's or Master's degree in Social Work, Psychology, Human Services, Healthcare, or related field
- Addiction certification/licensure (CADC or CAADC)
- Minimum 2 years experience with substance use disorders and co-occurring disorders
- FBI-DHS Clearance, PA State Criminal Check, PA Child Abuse Check
