Key Responsibilities
- Effectively communicates with participants, families, staff and other agencies and departments.
- Trained to drive all variety of vehicles that are included in the organization’s transportation fleet.
- Drives a specially equipped van to deliver physically limited persons to varied destinations.
- Affirms that wheelchair lifts and tie down equipment are properly operated and that participants are delivered to appointments in a timely manner.
- Follows established schedules, adjusts schedules to meet unanticipated changes, and determines the safest and most efficient pick-up and delivery routes.
- Assists participants on and off vans using proper safety techniques in a courteous manner.
- Demonstrates the proper use, care, and storage of durable medical equipment at all times.
- Makes sure to assist participants to and from their residence using appropriate safety measures related to the participant’s fall risk.
- Ensures participant exits residence safely and enters residence safely prior to leaving the residence.
- Responds to emergencies appropriately.
- Inspects van and maintains it in a clean, orderly and safe condition.
- Reports the need for maintenance.
- Completes daily maintenance check off sheet and submits to Transportation Coordinator or designee as assigned.
- Maintains proper documentation of fuel receipts as instructed by Transportation Coordinator and/or designee.
- Participates as a member of the Interdisciplinary Team as requested.
- Attends staff meetings and takes part in participant care planning as requested.
- Communicates any changes in participant condition to appropriate team members.
- Completes documentation as required.
- Maintains a safe driving record as evidenced by retaining a North Carolina driver’s license.
Requirements
- Must be 18 years old and have a minimum of 1 year of experience working with the frail elderly population.
- Valid NC Driver's License with good driving record with a minimum of no major moving violations.
- Special Training: Must be able to work effectively in a team environment and operate handicap accessible vehicles.
Qualifications
- Education: High School Diploma or GED.
