Responsibilities
- Collaborates with health care team to identify alternative placements for patients.
- Interfaces with members of the health care team, patients, families and vendors, while making referrals and transfer arrangements.
- Extracts supplemental information from the medical record in order to make appropriate referrals to facilities.
- Documents progress of the discharge planning process in the medical record and in the MSM system.
- Affords assistance with completion of pertinent screenings and contacts appropriate regulatory agencies.
- Facilitates referrals by telephone and/or fax to rehabilitation, skilled-nursing, or home care facilities.
- Arranges for durable medical equipment (DME) as directed by care coordination team.
- Identifies and recommends solutions regarding systems discharge issues.
Qualifications
- Bachelor’s degree in health services administration or related field.
- One to three years of experience in health care preferred.
- Excellent communication, collaboration, and organizational skills.
- Knowledge of a variety of computer-based software programs such as Word, Excel, etc.
Physical Demands And Environmental Conditions
Work is considered sedentary. Position requires work indoors in a normal office environment.
Pay
Hiring Range: $15.00 - $27.00. The final offer may vary within this range based on a candidate’s experience, skills, qualifications, and internal equity considerations.
Schedule
Monday through Friday, Weekends (Saturday and Sunday). Scheduled Hours: 8-430. Shift: 1 - Day Shift, 8 Hours (United States of America). Hours: 40.
