About the role

A Community Bridges Inc. (CBI) Discharge Supervisor oversees all aspects of discharge for patients receiving services in Crisis, Inpatient, or Transition Point programs. The Discharge Supervisor is actively involved in patient discharge planning from the time of admission and ensures that family members, enrolled providers, and other identified supports are engaged in the process. They attend 4-hour staffings and daily huddles with the multidisciplinary team to identify potential discharge barriers and provide feedback on the current discharge plan. The Discharge Supervisor ensures the discharge transition plan accurately reflects the discharge plan, including placement name, address, outgoing referral, and follow-up appointments. They work directly with the Clinical Manager, Clinical Director, and Operations team to ensure discharge planning and services are effective, efficient, and aligned with CBI Clinical Policy and Procedure, Arizona Administrative Code, ADHS/DBHS Provider Manual, Code of Federal Regulations, and contract requirements.

Responsibilities

  • Oversee all aspects of patient discharge planning in Crisis, Inpatient, or Transition Point programs.
  • Engage family members, enrolled providers, and other identified supports in the discharge process from admission.
  • Attend 4-hour staffings and daily huddles with the multidisciplinary team to identify discharge barriers and provide feedback.
  • Ensure the discharge transition plan accurately reflects placement details, outgoing referrals, and follow-up appointments.
  • Collaborate with the Clinical Manager, Clinical Director, and Operations team to align discharge services with organizational and regulatory standards.

Requirements

  • Bachelor’s Degree in a related field (Master’s Degree preferred).
  • 1-3 years of full-time behavioral health care experience (exposure to behavioral and medical crisis situations preferred).
  • Fingerprint clearance card.
  • Current Arizona Driver’s License.
  • Clear 39-month motor vehicle record.
