Responsibilities
- Provide individual and group treatment for identified MAT patients scheduled to release into the community, while also responsible for discharge planning in accordance with policy and procedures.
- Maintain relationships with community agencies for individuals with mental illness or serious medical conditions, and facilitate applications of eligibility for financial assistance.
- Care for medication and medical needs upon release, including potential residential placement, and assist inmates in making appointments with mental health agencies.
- Communicate and maintain positive working relationships with appropriate Unit Team Staff and Institutional Parole Officers, documenting all services provided to the inmate in the health record.
- Attend meetings relevant to release planning as directed by the Mental Health Coordinator, Clinical Supervisor or Mental Health Director, and comply with employee standards of the Facility.
Requirements
- Master’s degree in social work or closely related field from an accredited college or university.
- Previous experience in developing successful transition plans involving community resources for persons with mental illness.
- Experience in social work case management in a mental health agency for mentally ill population.
- Maintain CPR certification, and any other certifications (such as First Aid) required by contract.
- Must be eligible for LMHC or LICSW licensure, and be eligible for CADC or LADC certification/licensure.
